Outreach Adult Day Program

Our goal is to encourage seniors to maintain their independence and stimulate mental, physical and social skills in a dignified manner.� The program will:

assist them to achieve and maintain their maximum level of functioning.
prevent premature and inappropriate institutionalization
provide respite and information for the adult's caregivers�

This program is offered to persons:

60 years of age or older
requiring�minimal assistance with activities of daily living
with mild Alzheimers or related dementia
needing socialization and stimulation opportunities�

�Hours of�Operation:

Tuesday to Friday from 9:30 a.m.�to 2:30 p.m.�

Throughout the year.

What is the cost?

Daily program fee is $18.00 /day - includes a 3 course lunch and 2 snacks.
